Leaving a gift in your will is an impactful way to support the people and programs that are close to you, by investing in the future of health care for our communities.
If you or a loved one has received compassionate care at Southlake Health, leaving a legacy gift can ensure that hundreds of other community members and families like yours will have access to leading edge care close to home for generations to come, just like you did.
Southlake Health Foundation will work with you or your financial advisor to facilitate giving that meets your personal and estate goals, while supporting the areas of Southlake that matter the most to you. You can give confidently knowing your gift will make an impact, and that you will be remembered and recognized for your generosity.
By naming Southlake Foundation as a beneficiary in your will, you can leave the hospital a percentage of your estate or a specific dollar amount. You can make a significant and meaningful gift that has little to no effect on your current lifestyle, or on the gifts you intend to leave to your family. Revising your existing will is a straightforward process, and a bequest (a gift in your will) provides the most flexibility and security should your personal circumstances change.

Gifts of life insurance
A life insurance policy is a powerful way to turn small annual payments into a transformational future gift.
You can give through insurance in two ways:
- Name Southlake Health Foundation as a beneficiary of your existing or new policy — your estate will receive a charitable tax receipt for the proceeds.
- Donate a paid-up policy or transfer ownership — and receive a tax receipt for the fair market value today, plus ongoing receipts for any premiums you continue to pay.
This is a flexible, accessible option that allows you to make a major impact — often for less than you think.
Southlake Foundation is pleased to assist you and your professional advisor by providing or reviewing sample bequest wording to ensure the Foundation can accept and use your gift as you intend. Please use the legal name “Southlake Health Foundation” in your bequest clause. Our Charitable Number is 13179 7540 RR0001.
