Make a memorial gift
Choose to honour someone’s memory with a personal donation. You can send a customized e-card or printed card to the family, letting them know a gift has been made in tribute to their loved one.
Have a custom memorial page created
Southlake Health Foundation can create a customized memorial page for your loved one. Reach out to our team with a photo (required) and obituary (optional) and we’ll create a custom landing page within 3 – 4 business days.
